To register for NCCER 2010 we ask that you:

• Complete and submit the following form.
• Submit your payment using one of these methods:
  • VISA or Mastercard - Please call the SERT office at 519-661-2111 ext. 84824 between 8am and 4pm.
  • Cheque - Please mail a cheque, made out to "The Student Emergency Response Team" and marked "ATTN: NCCER" on the envelope, to our office (The Student Emergency Response Team, Room 28, The University Community Centre, The University of Western Ontario, London, Ontario, Canada, N6A 3K7). Note: Cheques must be post-marked prior to the date of rate expiry in order to receive the discounted rate.

The cost to attend NCCER 2010 is dependent on the day you register, so register early and save!

• Jan 24th – Jan 31st: $90
• Feb 1st – Feb 7th: $95
• Feb 8th – Feb 17th: $100

Note to all teams: In order to ensure that your competition teams are registered successfully please submit an additional payment of $50 per team using one of the aformentioned payment methods.
